Connecting YOUth Program

 The Connecting Youth Program helps youth ages 14-17 build a support system so they have life-long connections within the community. The program helps youth find family or people they consider as family, and mentors to help them transition into adulthood. Youth receive assistance in high school, help finding employment, planning a career and applying to college or vocational school.

Services Include:

  • Prevention education
  • Flexible life skills training
  • Social development and leadership training
  • Family searches
  • Recreation and cultural activities
  • Community service and engagement
  • Educational advocacy
  • Mentoring

Connect to Home Apartments for young adults ages 18-21

Connect to Home will provide Housing Choice Vouchers through the Department of Children and Families and the Department of Community Affairs.

Services Include:

  • Life Skills Assessment and training on needed life skills
  • Medical and mental health care
  • Recreation and cultural activities
  • Advocacy and referrals
  • Individual counseling and case management
  • Job development and education assistance
  • Community service and engagement
  • Aftercare

Building Lasting Connections for youth ages 14 to 24

Youth referred by the Division of Child Protection and Permanency will receive assistance in building connections with family and the community.
